Job Description
The Care Coordination Representative is responsible for completing tasks related to assigned Care Coordination Services (CCS) / Health Information Management (HIM) services. CCS services include e-Filing (electronic filing), order and referral management, insurance verifications, prior authorizations, and completing patient medical forms. The Care Coordination Representative reports directly to the Operations Supervisor. This position may be onsite, hybrid, and/or remote, allowing you to work from your own home office environment.
Duties and Responsibilities
- Process assigned tasks with a focus on accuracy, efficiency, and compliance with client protocols.
- Evaluate medical documents and file them electronically into patient charts.
- Navigate client electronic health records (EHR) software and internal systems with ease.
- Handle high volumes of work while maintaining high quality and accuracy.
- Utilize strong data entry and typing skills with the ability to work quickly on a keyboard.
- Crosstrain in multiple EHR systems to provide additional team coverage when needed.
- Follow predefined filing processes and refer to internal documentation or supervisors when assistance is needed.
- Understand and utilize assigned client protocols and Service Level Agreements.
- Track completed work, logging time and transaction counts as directed by management.
- Know the Planning Guidelines or target transactions per hour for assigned client accounts.
- Complete all work as assigned by management.
- Report to management any variances to standard performance, including delays or issues affecting job completion.
- Study and reference internal documentation and protocols regularly.
- Maintain confidentiality and follow HIPAA guidelines; report any HIPAA violations.
- Provide exemplary customer service to customers, vendors, visitors, coworkers, and management.
- Attend required meetings and training sessions.
- Understand and comply with company and client compliance standards.
- Protect sensitive data as required by the role.
- Perform other duties as assigned to support department and company operations.
Minimum Qualifications
- High School Diploma or equivalent required.
- Experience with health information management concepts, EHR systems, or medical terminology is a plus.
- Ability to manage a high-volume, fast-paced environment with accuracy.
- Intermediate computer skills, including Microsoft Office Suite and adaptability to company-specific software.
- Ability to efficiently navigate multiple programs, windows, and applications.
- Strong attention to detail and accuracy.
- Ability to perform physical tasks such as sitting, talking, hearing, using hands, reaching, standing, walking, driving, and occasionally lifting up to 25 lbs.
- Occasional after-hours or weekend work may be required.
- Ability to work remotely with reliable internet access in a secure and private workspace.
